If the patient is seeking a physical to participate in a GISD activity, please bring GISD’s physical packet to the appointment, completed and signed by a parent or guardian (English).

For Covid-19 vaccination of a minor, please complete the Covid-19 Vaccine Parental Consent Form (English) (Spanish). This form can also be completed in Skyward Family Access

If you or your child are coming in for Covid testing, complete the Covid Testing and Consent Form (English) (Spanish).

Please bring the completed form to your appointment or arrive a few minutes early to fill them out at the clinic of your choice.

To refer someone for mental health therapy, including your own child, call Teen Health Center at (409) 761-3530 ext. 7, complete and deliver the Mental Health Referral Form (English) (Spanish) to any clinic, or fax it to 409-765-5026.

If you are a teacher or other GISD staff member who would like to make Social and Emotional Learning Referral, please complete the Campus Based SEL and Mental Health Referral Form (English) and submit it to your school counselor.
