What To Know Before Your Visit
Before you visit, parents of patients under the age of 18 should complete a guardian consent form to bring to the visit. If an older child is attending an appointment without a parent or guardian they can bring the parental consent form or it may be emailed to records@teenhealthcenter.org.

Parent or guardian photo ID
Immunization records
Current medications
Medical history
Completed Parent Authorization Form (if child is under 18)
Confidential Care for Teens
As children grow into adolescence, developing trust with their healthcare provider becomes increasingly important. In certain situations allowed by law, teens may receive confidential care without a parent present. We encourage open communication within families while also providing a safe environment where young people can discuss sensitive health concerns. If there is concern that a student may be at risk of harming themselves or others, parents or guardians will be notified immediately.
